By completing this online application I authorize the following:
- That my participation in the Circleplay, Inc. women's wellness and fitness program is completely voluntary. During the course of attending these sessions, I will receive information and direction about personal fitness and wellness. I am fully aware that this program will require physical exertion which may be strenuous and physical injury could result. I am fully aware of the risks and hazards involved.
- I agree to consult with my physician prior to participating. My signature and attendance warrant that I am physically fit and I have no medical condition that would prevent my participation in this program.
- I agree to assume full responsibility for any injuries, risks or damages that I may incur as a result of my participation in the program.
- I knowingly and voluntarily waive any claim against Circleplay, Inc. for injury or damages I may sustain as a result of my participation in the program.
- I, my heirs, or legal representatives forever release, waive, discharge, and covenant no to take any legal action against Circleplay, Inc. for any injury or dealth caused by their negligence or other acts.
- I authorize Circleplay, Inc. to use my photograph in promotional materials without regard for any payment or consideration made to me.
